About this test

An Extra Large Biopsy refers to histopathological examination of a sizeable piece of tissue โ€” typically a whole organ segment, a major lump, or a wide surgical resection rather than a small punch or needle core. After your surgeon removes the specimen, our pathologists weigh and measure it, photograph and ink the margins, then dissect it carefully so that representative blocks are taken from the tumour, its edges, and any nearby lymph nodes. These blocks are processed overnight, embedded in paraffin wax, sectioned thinly, stained (usually with haematoxylin and eosin), and studied under the microscope. The report tells your treating doctor exactly what the tissue is, whether it is benign or malignant, the grade and type of any tumour, how deeply it has invaded, and โ€” crucially for cancer surgery โ€” whether the disease has been completely removed or reaches the cut margins. This category covers larger excisions such as mastectomy specimens, bowel or stomach resections, uterus and ovaries, gallbladder with mass, thyroid lobes, and limb or soft-tissue masses. Because the volume of tissue is greater and may need extra sampling, decalcification of bone, or additional sections, the turnaround is longer than for small biopsies. In Nepal, where many patients reach surgery with advanced disease, this examination is often the single most important step in confirming a diagnosis, staging a cancer, and deciding whether chemotherapy or radiotherapy is needed. Where the picture is uncertain, special stains or immunohistochemistry may be added, occasionally extending the reporting time.
