About this test

A skin biopsy involves removing a small piece of skin so that a pathologist can examine it under the microscope and give a definitive diagnosis that the naked eye alone cannot provide. The tissue is fixed in formalin, processed, sliced into thin sections, stained and read by a consultant, who reports the architecture of the epidermis and dermis, the pattern of inflammation, and the nature of any abnormal cells. Your doctor may order it for a mole or growth that is changing in size, colour or shape, for a persistent ulcer or non-healing sore, or for a stubborn rash that has not responded to treatment and needs its underlying mechanism clarified. It is the only reliable way to confirm or exclude skin cancers such as basal-cell carcinoma, squamous-cell carcinoma and melanoma, and it distinguishes between conditions that look alike clinically โ€” psoriasis versus eczema, lichen planus, lupus, or a drug reaction. In Nepal, where high-altitude ultraviolet exposure and outdoor farming work raise the risk of sun-related skin damage, biopsy plays an important role in clarifying suspicious lesions early. It is also frequently used here to investigate chronic skin infections, including cutaneous tuberculosis and leprosy, which remain part of everyday dermatology in this region and often require tissue confirmation alongside special stains. The microscopic findings are interpreted together with your clinical history, the appearance and site of the lesion, and sometimes additional immunohistochemical or culture studies, allowing your treating doctor to plan the correct treatment with confidence.

Preparation

No fasting needed. The sample is taken by your doctor or dermatologist under local anaesthetic, so the skin lesion is numbed first. Tell the doctor if you take blood-thinning medicines (aspirin, warfarin, clopidogrel) or have a bleeding tendency. Keep the dressing dry and follow wound-care advice until your results are ready.
