About this test

When a surgeon removes a sizeable piece of tissue โ€” a lump, an organ segment such as part of the bowel, breast, uterus, or a wide excision of a skin or soft-tissue growth โ€” that specimen comes to the laboratory for histopathological examination, and this is what a large-specimen biopsy report covers. Unlike a small needle core, a large specimen lets the pathologist assess the whole lesion: its exact nature (benign or malignant), the tumour type and grade, how deeply it has invaded, whether it involves blood vessels or nerves, and crucially whether the surgical margins are clear of disease. The tissue is fixed in formalin, cut into sections, processed overnight, embedded in paraffin wax, sliced thinly, stained, and studied under the microscope; larger specimens need more blocks and careful sampling, which is why the report takes several working days. Your doctor relies on this report to confirm a diagnosis, to stage a cancer, and to decide whether further treatment โ€” chemotherapy, radiotherapy, or repeat surgery โ€” is required, so it is read alongside your imaging, blood results, and operative findings. In Kathmandu this examination is central to managing the gastric, cervical, gallbladder, and oral cancers that are common here, as well as confirming tuberculosis in resected lymph nodes or intestine. Where the diagnosis is uncertain, we may add special stains or immunohistochemistry, which can extend the turnaround.
