About this test

The Indirect Coombs Test looks for antibodies that are floating freely in your blood serum and are capable of attacking red blood cells that are not your own. Unlike the direct version, which examines cells already coated with antibody, this test mixes your serum with carefully selected donor red cells and then adds an antiglobulin reagent to see whether clumping occurs. Its two everyday uses are in pregnancy and in transfusion safety. During antenatal care it screens for anti-D and other irregular antibodies in mothers, particularly those who are Rh-negative; if a Rh-negative mother carries a Rh-positive baby, her immune system can form antibodies that cross the placenta and destroy the foetus's red cells, causing haemolytic disease of the newborn. Detecting these antibodies early lets doctors plan anti-D prophylaxis and monitor the pregnancy closely, which matters a great deal in Nepal where many women present late for antenatal screening. The same test underpins the cross-match before any blood transfusion, ensuring donor blood will not trigger a dangerous reaction in the recipient. It is also ordered when investigating an unexplained transfusion reaction or unexpected anaemia. A negative result is reassuring; a positive result prompts antibody identification to pinpoint exactly which red-cell antigen the antibody targets. Results are always read alongside your blood group, Rh type, haemoglobin, and reticulocyte count, and in pregnancy alongside the antibody titre, which is repeated through the trimesters to track whether levels are rising.
