About this test

D-Dimer is a tiny protein fragment left behind when the body breaks down a blood clot, so its level in the blood rises whenever clotting and clot-dissolving are happening somewhere inside you. Doctors order this test mainly to help rule out two dangerous conditions: a clot in the deep veins of the leg (deep-vein thrombosis) and a clot that has travelled to the lungs (pulmonary embolism). Its real strength lies in a normal result โ€” when D-Dimer is low in a patient judged to be at low or moderate risk, a clot becomes very unlikely, which can spare you from a CT scan or other heavier imaging. A raised value, on the other hand, is not by itself proof of a clot; it simply means more testing is needed, because levels also climb with infection, recent surgery, injury, pregnancy, liver disease, cancer, and even advancing age. For this reason your doctor never reads it in isolation but alongside your symptoms, a clinical risk score, and often a Doppler ultrasound or CT angiogram. The test is commonly requested when someone arrives with sudden one-sided leg swelling and pain, unexplained breathlessness, chest pain that worsens on breathing in, or a racing heart. It is also used to monitor disseminated intravascular coagulation, a serious clotting disturbance that can complicate severe infection or pregnancy. In Kathmandu we see it requested frequently after long flights and bus journeys, during high-risk pregnancies, and in hospitalised or post-operative patients who have been immobile.

No fasting required. Drink water normally. If you are on blood-thinning medicine such as warfarin, heparin, or a newer anticoagulant, please tell us, as it can affect how the result is read.
