About this test
Erythropoietin is the hormone your kidneys release to tell the bone marrow how many red blood cells to make. This test measures the amount of EPO circulating in your blood, and its real value lies in interpreting it against your haemoglobin and haematocrit rather than in isolation. When red-cell levels fall, healthy kidneys should respond by raising EPO; a low or inappropriately normal level in an anaemic patient points towards the kidney as the source of the problem, which is why doctors frequently order it in chronic kidney disease and before starting EPO-stimulating injections. The test works equally hard at the opposite end: when haemoglobin is unexpectedly high (polycythaemia), it helps separate primary marrow disorders such as polycythaemia vera โ where EPO is typically suppressed โ from secondary causes like chronic lung disease, heart disease, sleep apnoea, smoking, or rare EPO-producing tumours, where the hormone is raised. It is also used to investigate unexplained anaemia and to monitor response in patients already on treatment. A point worth noting locally: people who live at or have recently spent time at high altitude across Nepal's hill and mountain regions naturally carry higher EPO and red-cell counts, so this context must be shared at the time of sampling to avoid a misleading result. Because levels are also influenced by recent transfusion and time of day, your physician will weigh the figure alongside a complete blood count, kidney-function tests, and your clinical picture before drawing any conclusion.

No special fasting is needed, but the sample is best drawn in the morning, as EPO levels follow a daily rhythm and dip later in the day. Tell us if you have recently travelled to high altitude or received a blood transfusion, as both affect the result.
