About this test
ESR is a simple, inexpensive, non-specific test that measures how quickly red blood cells settle in a thin column of blood over one hour. A raised value tells the doctor that inflammation, infection, or tissue damage is happening somewhere in the body — but not where. That non-specificity is actually its strength: ESR is used as a broad screening test in patients with vague symptoms like fatigue, low-grade fever, weight loss, or joint aches, to decide whether something organic is going on. It is also the standard monitoring test for known inflammatory conditions like rheumatoid arthritis, tuberculosis, polymyalgia rheumatica, and giant-cell arteritis. ESR rises slowly over days and falls slowly over weeks, so it is often paired with CRP, which responds within hours and gives a more current picture of inflammation.
