About this test

Fibrinogen, also known as Factor I, is the protein your liver makes that sits at the very end of the clotting cascade — when you bleed, an enzyme called thrombin converts it into fibrin, the mesh that holds a clot together. This test measures how much working fibrinogen is circulating in your plasma, usually by the Clauss method. A doctor orders it for two quite different reasons. The first is to investigate bleeding: if you bruise easily, bleed for a long time after a small cut or dental work, or have a prolonged PT or APTT, a low fibrinogen may explain it. Levels fall in advanced liver disease, in inherited deficiencies, and dangerously fast in disseminated intravascular coagulation (DIC), where it is one of the key markers watched alongside platelet count, D-dimer and the clotting times. It is also checked before major surgery and is closely monitored in obstetric emergencies such as postpartum haemorrhage and placental abruption, where rapid replacement can be life-saving. The second reason is the opposite — fibrinogen is an acute-phase protein, so it rises with infection, inflammation, pregnancy, smoking and chronic disease, and a persistently raised level is one of several markers linked to higher cardiovascular risk. Because it behaves in both directions, the result is never read in isolation; your clinician interprets it together with a full coagulation screen, the clinical picture, and often a CBC. The sample must be taken in the correct citrate tube and processed promptly, which is why we report it the same day.
