About this test

Haemoglobin electrophoresis by HPLC separates and measures the different types of haemoglobin inside your red blood cells, allowing us to identify inherited haemoglobin disorders. A healthy adult carries mostly HbA, a small amount of HbA2, and only a trace of foetal haemoglobin (HbF). This test quantifies each fraction precisely and detects abnormal variants such as HbS (sickle cell), HbE, HbD and others, while a raised HbA2 points towards beta-thalassaemia trait. Doctors most commonly order it to investigate a microcytic, hypochromic anaemia that has not responded to iron, to explain a family history of thalassaemia or sickle cell disease, or as part of pre-marital and pre-pregnancy carrier screening so that couples understand the risk of passing a serious blood disorder to their children. It is also used to confirm a diagnosis when a child presents with persistent pallor, poor growth, an enlarged spleen, or repeated need for transfusion. The result is always interpreted alongside a complete blood count, red-cell indices (especially a low MCV and MCH) and iron studies, because iron deficiency can lower HbA2 and mask a thalassaemia trait. In Nepal, beta-thalassaemia and HbE are genuinely common, particularly in the Tarai and among certain Janajati and Madhesi communities, which is why this test carries real public-health value. A normal result is reassuring; an abnormal one is not a cause for alarm but a prompt for genetic counselling and, where relevant, testing of close family members.
