About this test
Haemoglobin is the iron-rich protein packed inside your red blood cells that carries oxygen from the lungs to every tissue and returns carbon dioxide for exhalation. This test reports the concentration of that protein in grams per decilitre of blood, and it is the single most direct measure of whether your blood can deliver enough oxygen. A doctor orders it to diagnose and grade anaemia, the most common reason being the persistent tiredness, breathlessness on mild exertion, pale skin, palpitations, headache, or cold hands that bring patients to clinic. In Nepal a low result is most often due to iron deficiency, which is widespread among menstruating women, growing children, and those following largely vegetarian diets where dietary iron is harder to absorb; intestinal worm infestation and chronic blood loss are other frequent local causes. The result is rarely read in isolation. It is interpreted alongside red-cell indices such as MCV and MCH, which separate iron-deficiency anaemia (small, pale cells) from vitamin B12 or folate deficiency (large cells) and from the inherited thalassaemia traits seen in some communities here. A high haemoglobin can reflect dehydration, chronic lung disease, smoking, or simply life at higher altitude. The test is also used to confirm safe levels before surgery, in pregnancy, before blood donation, and to monitor recovery during iron therapy. Standard cut-offs for anaemia are roughly below 13 g/dL in men and below 12 g/dL in non-pregnant women, though your doctor will judge your figure against your age, sex, and symptoms.
