About this test
Unlike the automated cell counts of a CBC, a peripheral blood smear puts a thin film of your blood under the microscope so that a pathologist can look directly at the size, shape, colour, and maturity of the cells themselves. This visual examination answers questions a machine cannot: it tells us whether red cells are small and pale (pointing to iron-deficiency anaemia, the commonest cause we see in Nepali women and children), large and oval (suggesting vitamin B12 or folate deficiency, frequent in strict vegetarian diets), fragmented, sickled, or shaped like teardrops. We also study the white cells for abnormal or immature forms that may signal leukaemia, and we estimate whether platelets are genuinely low or merely clumped in the tube. The smear is the single most important step in identifying malaria, where the parasite is seen living inside the red cells, and it remains essential in any febrile patient during the monsoon season or after travel to the Terai. Doctors typically order a blood film when a CBC shows an unexplained abnormality, when someone has persistent fatigue, jaundice, unusual bruising, swollen glands, or a fever that has not been explained, and when a haematological cancer is suspected. The findings are always interpreted together with your CBC numbers and clinical picture, and in some cases they prompt confirmatory tests such as a reticulocyte count, iron studies, or a bone-marrow examination. It is a quick, inexpensive, and remarkably informative window into your blood.
