About this test
Prothrombin Time measures how many seconds your blood takes to form a clot through the extrinsic and common coagulation pathways, and the INR converts that figure into a standardised ratio so results mean the same thing from one laboratory to another anywhere in the world. The test is most often ordered to monitor people taking warfarin, the oral blood-thinner widely prescribed in Nepal after heart-valve replacement, for atrial fibrillation, or following a deep-vein thrombosis or pulmonary embolism โ the dose is adjusted to keep the INR within a target range (commonly 2.0 to 3.0, or higher for mechanical valves), balancing the risk of clotting against the risk of bleeding. Doctors also request it to investigate unexplained bruising, prolonged bleeding from cuts or gums, or heavy menstrual loss, and to assess liver function, since the liver makes most clotting factors and a rising PT/INR can be an early sign of significant liver damage from hepatitis or alcohol. It is a routine check before surgery, before a liver biopsy, and in newborns or malnourished patients where vitamin K deficiency is suspected. The result is rarely read alone: it is interpreted alongside the APTT, platelet count, and sometimes fibrinogen and D-dimer to build a full picture of how your blood clots. A prolonged time simply means clotting is slower than expected and always needs to be matched against your medicines, symptoms, and overall clinical picture rather than judged on the number by itself.
