About this test
Reticulocytes are young red blood cells that have just left the bone marrow and still carry faint remnants of the machinery they used to make haemoglobin. By counting what fraction of your circulating red cells are this fresh, the reticulocyte count tells your doctor one crucial thing that a haemoglobin level alone cannot: how hard your bone marrow is currently working to replace red cells. It is the single most useful follow-up test when a Complete Blood Count shows anaemia, because it sorts the cause into two camps. A low or inappropriately normal count points to a marrow that is not producing enough, as in iron, vitamin B12 or folate deficiency, chronic disease, kidney failure, or marrow suppression. A high count signals a marrow responding briskly, which happens after recent blood loss, during recovery once iron or B12 has been started, and in conditions where red cells are being destroyed prematurely such as haemolytic anaemia, thalassaemia, or G6PD-related haemolysis. In Kathmandu, where iron-deficiency anaemia and inherited haemoglobin disorders are both common, this test is frequently used to confirm that prescribed iron is actually working, with a rising count within a week or two being the earliest sign of a good response. Doctors interpret it alongside your haemoglobin, MCV, peripheral smear, and iron or vitamin studies, and may report it as a percentage or, more usefully, as an absolute count or corrected index. The sample is a small tube of blood, and results are available the same day.
