About this test

The Total Leucocyte Count tells your doctor how many white blood cells are circulating in a given volume of your blood โ€” the standing army your body uses to fight infection. In most healthy adults the figure sits between roughly 4,000 and 11,000 cells per microlitre, and movement outside that window is one of the quickest clues to what is happening inside you. A raised count (leucocytosis) most often points to a bacterial infection, a localised abscess, recent tissue injury, or physical stress, while very high readings occasionally signal a blood disorder such as leukaemia that needs urgent follow-up. A low count (leucopenia) can follow viral fevers, typhoid (a common culprit in Kathmandu), certain medicines, or a bone-marrow problem. This is why doctors here so frequently order a TLC when someone presents with fever, chills, a sore throat, a urinary or chest infection, an unhealing wound, or unexplained tiredness โ€” and routinely before surgery to confirm there is no hidden infection. On its own the TLC only gives the headline number; its real value emerges when read together with the differential count (which white cells are raised), the platelet count, haemoglobin, and inflammatory markers like CRP. A single abnormal result is rarely alarming by itself, because counts vary with the time of day, pregnancy, smoking, and even a recent meal or exercise, so your doctor will interpret it alongside your symptoms and may simply repeat it after a few days to confirm a genuine trend.
