About this test

This test measures the total amount of calcium your kidneys pass into the urine across a full 24-hour day, which is a far more reliable picture than a single random sample because calcium excretion swings up and down with meals and the time of day. The commonest reason a doctor in Kathmandu orders it is to investigate recurrent kidney stones — the majority of stones contain calcium, and many stone-formers turn out to be quietly losing too much calcium in their urine (hypercalciuria) despite a normal blood level. It is also used to look into the cause of a high or low blood calcium, to assess bone-thinning conditions and osteoporosis, to monitor people on long-term vitamin D or calcium supplements, and to help distinguish primary hyperparathyroidism from a harmless inherited condition called familial hypocalciuric hypercalcaemia, where the two look almost identical on blood tests but the urine result tells them apart. Diet matters too — a very high salt or animal-protein intake pushes calcium losses up, so your doctor will interpret the figure in light of how you normally eat and any medicines you are taking.

Preparation

No fasting needed, but the collection must be complete. On the morning you start, empty your bladder into the toilet and note the time — then collect every drop of urine for the next 24 hours into the container we provide, finishing with one final sample at the same time the next morning. Keep the container cool and bring it in promptly. Tell us about any calcium or vitamin D supplements, thiazide diuretics, or antacids you take, as these change the result.
