About this test

This test measures the total amount of protein your kidneys leak into the urine across a full day, giving a far more reliable figure than a single random sample because protein loss naturally rises and falls between waking, eating and resting. Healthy kidneys hold protein back in the bloodstream and allow only a tiny trace (well under 150 mg) to escape in 24 hours; when the filtering units are damaged, larger amounts spill through, so a raised result is one of the clearest early signals of kidney disease. Your doctor most often orders it when a dipstick has flagged protein, when swelling appears around the ankles, eyes or face, when urine looks persistently frothy, or to monitor known diabetes and high blood pressure — the two commonest drivers of chronic kidney disease in Nepal, where both are rising sharply and often go undetected for years. It is also a cornerstone test in pregnancy: in a woman past 20 weeks with raised blood pressure, heavy protein loss helps confirm pre-eclampsia, a condition that needs prompt attention for both mother and baby. The result is rarely read alone. We interpret it alongside serum creatinine, eGFR, blood albumin, blood pressure and sometimes a kidney ultrasound to judge how well the kidneys are working and how fast any damage is progressing. Very high levels (the nephrotic range) point towards conditions affecting the filtering membrane itself. Repeating the test over time is one of the best ways to track whether treatment for diabetes, hypertension or glomerular disease is protecting your kidneys.

Preparation

Collect every drop of urine over a full 24-hour period in the container we provide. On the morning you begin, empty your bladder into the toilet and note that time — then collect all urine from then until the same time the next morning, including that final sample. Keep the container cool and out of direct sunlight, and avoid heavy exercise during the collection day as it can falsely raise the result.
