About this test

The white blood cell count tallies the body's defending cells circulating in a small tube of blood, typically expressed in thousands per microlitre. White cells are the immune system's frontline army, and their total number shifts in a remarkably telling way: a raised count (leucocytosis) most often signals a bacterial infection, an abscess, or inflammation, while it can also rise during physical stress, after steroid use, or in certain blood cancers such as leukaemia. A low count (leucopenia) may point to a viral illness, an overwhelming infection, a vitamin deficiency, an autoimmune condition, or the marrow-suppressing effect of chemotherapy and some medicines. A doctor usually orders it when you arrive with persistent fever, chills, a sore throat that will not settle, unexplained fatigue, recurrent infections, or unusual bruising. In Kathmandu it is a routine companion to fever workups during monsoon season, helping distinguish bacterial enteric fever from the typically normal or low counts seen in dengue and other viral fevers, and it is checked before surgery and during follow-up of patients on immune-affecting treatment.
