About this test

This test looks for tiny amounts of albumin, a blood protein, leaking into the urine in quantities far too small for an ordinary dipstick to catch. Healthy kidneys hold albumin back, so even a modest leak is an early warning that the filtering units are under strain. When measured against urine creatinine it gives the albumin-to-creatinine ratio (ACR), a value of 30 to 300 mg/g signalling microalbuminuria and anything above suggesting more established kidney damage. A first-morning sample is preferred because it is more concentrated and avoids the false rises that upright daytime activity can cause. The most common reason your doctor orders it is to screen for the earliest kidney effects of diabetes and high blood pressure, both rising sharply across Nepal and often present silently for years before any symptom appears. Detecting albumin at this stage matters because the change is frequently reversible with better sugar and blood-pressure control, medicines that protect the kidney, and lifestyle adjustment. People with diabetes are usually tested yearly even when they feel completely well; you may also be asked to repeat it because a single raised value can be triggered by fever, infection, heavy exercise, or menstruation rather than true kidney disease. The result is read alongside your blood sugar, HbA1c, serum creatinine and eGFR, and blood pressure to build a full picture of kidney health. Because confirming microalbuminuria usually needs two or three positive samples over a few months, an early test is the start of monitoring, not a final verdict.

Preparation
