About this test

Microalbumin measures very small amounts of albumin — a protein that healthy kidneys normally hold back in the blood — leaking into the urine in quantities too low for an ordinary dipstick to detect. On a random spot sample we usually report it as an albumin-to-creatinine ratio (ACR), which corrects for how dilute or concentrated the urine is and removes the need for an awkward 24-hour collection. A persistently raised ACR is the earliest reliable warning that the delicate filtering units of the kidney are being damaged, often years before blood creatinine rises or any symptom appears. Its main use is screening and monitoring people with diabetes and high blood pressure, the two commonest causes of chronic kidney disease in Kathmandu, where rising rates of type 2 diabetes make early detection particularly valuable. Preparation

No fasting needed. A single spot sample of urine is enough — early-morning or mid-morning is ideal. Avoid testing during a fever, urinary infection, heavy menstrual bleeding, or within 24 hours of vigorous exercise, as these can falsely raise the result.
