About this test
Microprotein measures the small amount of protein leaking into your urine โ quantities far too low to be picked up by an ordinary dipstick. Healthy kidneys act as a fine sieve, holding protein back in the blood while filtering out waste, so when even traces of albumin and other proteins appear in urine it is one of the earliest warning signs that the filtering units are under strain. The test is most often ordered as part of routine surveillance in people with diabetes and high blood pressure, the two leading causes of chronic kidney disease, where catching protein loss early can mean treatment begins long before kidney function visibly declines. Your doctor may also request it if a dipstick was unexpectedly positive, if you have unexplained swelling of the ankles or around the eyes, frothy urine, or as part of investigating preeclampsia in pregnancy. Results are usually expressed as a concentration or, more reliably, as a protein-to-creatinine ratio on a spot sample or a total over a timed 24-hour collection, which corrects for how dilute or concentrated the urine happens to be. A single raised value does not by itself confirm disease โ fever, vigorous exercise, urinary infection and dehydration can all transiently push protein up, so an abnormal result is typically repeated and read alongside blood creatinine, eGFR, blood sugar and blood pressure. In Nepal, where diabetes and hypertension are rising steeply and kidney disease often goes undetected until late, this simple, painless test is a valuable tool for protecting kidney health while there is still time to act.

Preparation
No fasting required. If your doctor has asked for a 24-hour collection, discard the first morning sample, then collect every drop for the next 24 hours in the container provided and keep it cool. Avoid strenuous exercise the day before, and mention if you are menstruating, as blood can affect the result.
