About this test
A PAP smear is a screening test that looks at the cells of the cervix โ the neck of the womb โ to catch abnormal changes long before they could ever become cancer. During a short pelvic examination a doctor gently brushes a few cells from the surface and the opening of the cervix; our cytologists then stain these cells using the Papanicolaou technique and examine them under the microscope for any departure from normal shape, size, or maturation. The test is designed to detect pre-cancerous changes (reported on the Bethesda system as ASC-US, LSIL, HSIL, and so on), as well as everyday findings such as infection, inflammation, yeast, bacterial vaginosis, and the cell changes caused by human papillomavirus (HPV), the virus responsible for almost all cervical cancers. Cervical cancer remains one of the commonest cancers among women in Nepal, largely because it is often found late โ yet it is one of the most preventable cancers we know of, precisely because this slow-growing disease gives a wide window in which screening can intervene. Most guidelines suggest beginning regular screening from around age 21 to 30 and repeating every three years if results are normal, though your doctor will tailor this to your history. A smear is usually interpreted alongside any HPV testing, your symptoms (such as bleeding between periods or after intercourse), and previous results. An abnormal report is common and rarely means cancer; it simply signals the need for a repeat test or a closer look by colposcopy.

Preparation
No fasting required. Book for a day when you are not menstruating โ ideally about 10โ20 days after the first day of your last period. For 48 hours beforehand, avoid vaginal medicines, creams, pessaries, douching, lubricants, and intercourse, as these can obscure the cells. Tell us if you are pregnant or breastfeeding.
