About this test

The urine protein creatinine ratio is a quick, convenient way to measure how much protein is leaking into your urine without the inconvenience of collecting every drop over 24 hours. From a single spot sample, the laboratory measures both protein and creatinine and divides one by the other; because creatinine is excreted at a fairly steady rate, this ratio corrects for how dilute or concentrated the sample happens to be and gives a reliable estimate of daily protein loss. Healthy kidneys keep protein in the bloodstream, so a persistently raised ratio is one of the earliest and most important signs that the kidneys' filtering units are damaged. Doctors order this test to investigate frothy or foamy urine, swelling of the ankles, face, or around the eyes, and to screen and monitor people living with diabetes and high blood pressure โ€” the two leading causes of chronic kidney disease in Nepal. It is also used to follow up an abnormal urine dipstick, to monitor pregnant women for pre-eclampsia, and to watch the kidneys during treatment with certain medicines. A ratio below about 0.2 mg/mg is generally considered normal, while higher values point towards conditions such as glomerulonephritis or diabetic nephropathy. The result is rarely read alone: your clinician will interpret it alongside serum creatinine and eGFR, blood pressure, blood sugar, and a urine microscopy. Because infection, fever, and strenuous activity can falsely elevate protein, a single high reading is usually confirmed on a repeat sample before any firm conclusions are drawn.

Preparation

No fasting needed. A random spot sample is fine โ€” early-morning urine is preferred but not essential. Avoid heavy exercise just before collection, and tell us if you are menstruating, have a fever, or have a urinary infection, as all can temporarily raise the result.
