About this test

A semen analysis examines a man's ejaculate under the microscope and against WHO laboratory standards to judge fertility potential. We assess volume, liquefaction time, pH and appearance, then measure the things that matter most for conception: sperm concentration (count per millilitre), total motility and progressive forward movement, and morphology — the proportion of sperm with a normal head, midpiece and tail. We also note the presence of white blood cells, which can hint at infection or inflammation in the reproductive tract. The most common reason this test is ordered is a couple who have been trying for a pregnancy for a year or more without success; because the cause of infertility lies with the man in roughly half of all couples, evaluating him is just as important as investigating the woman, and a semen analysis is the simplest, least invasive place to start. Doctors also use it to confirm success after a vasectomy, to follow up treatment for varicocele or hormonal problems, and sometimes before fertility-preservation freezing. Results are best interpreted alongside hormone tests such as FSH, LH and testosterone, and a clinical examination. A single low or abnormal result does not mean infertility — sperm production is sensitive to recent fever, illness, alcohol, smoking, heat, and stress, so we usually recommend repeating the test after a few weeks before drawing any firm conclusion. Because sample quality falls quickly with delay, fresh collection and prompt examination give the most reliable picture.

Preparation

Collect the sample after 2–5 days of sexual abstinence — not less than 2 and not more than 7. Produce the sample by masturbation into the sterile container we provide, ideally in the room at our lab so it can be examined while fresh. If collected at home, keep it close to body temperature and deliver it within one hour, and write down the time of collection. Do not use condoms, lubricants, or saliva. Tell us about any recent fever, illness, or medication.
