About this test

Urine acetone detects ketone bodies — chiefly acetone, acetoacetate and beta-hydroxybutyrate — which spill into the urine when the body burns fat instead of glucose for energy. Normally there are no detectable ketones; their appearance signals that fat breakdown has accelerated. The commonest reason a doctor orders this test is uncontrolled diabetes: in a person with type 1 diabetes, or anyone with very high blood sugar, vomiting, deep rapid breathing, abdominal pain or a fruity smell on the breath, a positive result can be the first warning of diabetic ketoacidosis, a dangerous emergency that needs immediate treatment. Beyond diabetes, ketones turn up during prolonged fasting or starvation, persistent vomiting, severe diarrhoea with dehydration, strict low-carbohydrate or "keto" diets, and the morning sickness of early pregnancy when a woman cannot keep food down. In children, who tip into ketosis quickly, the test helps explain vomiting and lethargy during a febrile illness or a missed meal. It is a simple, inexpensive screen — often done on a dipstick — and is almost never read alone: your doctor interprets it alongside blood glucose, blood gases, and electrolytes to judge how severe the situation is. Because acetone evaporates readily and the chemical strips react mainly to acetoacetate, a fresh sample matters, and in a sick diabetic a blood ketone (beta-hydroxybutyrate) measurement may be added for accuracy. A trace of ketones after an overnight fast can be harmless, but a strongly positive result, especially with symptoms or known diabetes, should be reviewed by a doctor without delay.
