About this test

This test looks for chyle — the milky, fat-rich lymphatic fluid that normally carries digested fats from the gut — leaking abnormally into the urine, a condition called chyluria. When this happens the urine turns pale and milky, sometimes thick enough to clot or layer into a creamy top with a clearer bottom on standing. In the laboratory we confirm true chyle by mixing the sample with ether or chloroform: genuine chyluria clears or thins as the fat dissolves into the solvent, separating it from other causes of cloudy urine such as pus, phosphates, or pyuria. We also check for accompanying protein and, often, microscopic red cells, since chyluria and intermittent blood in the urine frequently appear together. The single most important reason a doctor orders this test in Nepal is lymphatic filariasis — chronic infection with Wuchereria bancrofti, still endemic in parts of the Terai and historically across the country — which damages the lymphatic vessels and lets chyle spill into the renal tract; the milky urine is often intermittent and worse after fatty meals. Other causes include lymphatic obstruction from tuberculosis, abdominal tumours, trauma, or surgery near the lymph channels. Patients usually notice it when their urine looks like diluted milk, sometimes with weight loss, tiredness, or low protein from the steady loss of fat and protein. The result is interpreted alongside a routine urine examination, night-blood film for microfilariae, and imaging of the urinary and lymphatic systems to pin down the underlying cause.

Preparation

No fasting needed for the basic test. If your doctor suspects chyluria, eating a fatty meal (milk, ghee, butter) a couple of hours before passing the sample can make the milky appearance more obvious and improve detection. Collect the sample in a clean, dry container and bring it to the lab promptly.
