About this test
Urine cytology is a microscope examination of the cells that naturally shed from the lining of the urinary tract โ the bladder, ureters, renal pelvis and urethra โ into a fresh urine sample. After the sample is spun down and stained, a pathologist studies the size, shape and nuclei of these cells to look for changes that suggest cancer, particularly urothelial (transitional cell) carcinoma of the bladder. A doctor most often orders it when a patient has painless blood in the urine (visible or picked up on a dipstick), persistent urinary irritation that has not settled with treatment, or repeated unexplained urinary infections, and it is a standard part of the work-up alongside cystoscopy and a CT urogram. It is also used to monitor people already diagnosed with bladder cancer for recurrence after treatment. The test is most reliable for high-grade tumours, where abnormal cells are easy to recognise; low-grade tumours shed cells that look almost normal, so a negative result never fully excludes disease and is always read in the context of the patient's symptoms and other investigations. In Nepal, where chronic infections, urinary stones and prior tuberculosis can all inflame the bladder lining and mimic worrying cell changes, an experienced cytopathologist is essential to separate reactive, infection-related changes from genuine malignancy. Smoking is the single biggest risk factor for the cancers this test detects, so let your doctor know your smoking history. Results are typically reported as benign, atypical, suspicious or positive for malignant cells.

Preparation
No fasting required. We prefer a freshly voided mid-stream sample, ideally not the very first urine of the morning โ a sample passed a little later in the day, after you have been up and active, yields better-preserved cells. Stay normally hydrated. If a course of urine cytology has been requested, bring a separate sample on each of the days advised, as repeat samples improve the chance of detecting abnormal cells.
