About this test

Urobilinogen is a colourless by-product formed when gut bacteria break down bilirubin, the pigment your liver excretes into bile. Most of it is reabsorbed, processed again by the liver, and a small amount spills into the urine — so a trace is entirely normal and healthy. This test, usually read off a urine dipstick during a routine urinalysis, measures whether that amount is raised or, less easily detected, absent. A high urobilinogen level points the doctor towards two broad situations: the liver is struggling to re-process what it reabsorbs (as in hepatitis, including the viral hepatitis A and E that follow contaminated water and food during Kathmandu's monsoon, alcohol-related liver disease, or cirrhosis), or red blood cells are being destroyed faster than normal (haemolysis), flooding the system with extra bilirubin. By contrast, a complete absence of urobilinogen suggests bile is not reaching the gut at all — a blocked bile duct from gallstones or a pancreatic-head tumour — and this is typically accompanied by pale, clay-coloured stools and dark urine. Because the dipstick value is non-specific on its own, your doctor interprets it alongside urine bilirubin, the pattern of liver enzymes and bilirubin on a liver function test, and a complete blood count if haemolysis is suspected. It is commonly ordered when someone develops yellowing of the eyes or skin (jaundice), unexplained fatigue, abdominal discomfort, dark urine, or during follow-up of known liver disease. The result is reassuringly quick and needs only a simple urine sample.
